Free income tax assistance for Seniors and low-to-moderate income taxpayers will be available February, March and April of 2024.

Starting Monday, January 15th, you can make an appointment by calling the library at 920-459-3400, or in person at any library Service Desk.  Appointments are made on a first-come, first-served basis. You will then need to fill out forms required by Tax-Aide. (Intake/Interview & Quality Review form 13614-C and WI Tax Aide Information Sheet)  You are encouraged to pick these up ahead of time at the first floor Service Desk.  If that isn't possible, you should arrive at least 20 minutes prior to your appointment so that they can be completed.

Please note: If your taxes are complicated and will require more time than the one hour appointment allows, Tax-Aide asks that you not use this service so that it may better serve needier taxpayers.

Bring a copy of last year’s (2022) tax return to your appointment, as well as all required 2023 documentation, including but not limited to:

-----Completed Intake/Interview and Quality Review form 13614-C,  and WI Tax-Aide Information Sheet.

-----Proof of Identity (Photo ID and Social Security card for everyone to be included on the tax return)

-----Bank account information (routing & account numbers if direct deposit or direct debit is wanted)

-----Copy of 2022 Federal and State tax returns

-----Wage and Earnings statement(s) [W-2]

-----Taxable Interest [1099-INT]

-----Social Security benefits [SSA-1099]/Railroad Retirement benefits [RRB-1099]

-----Pension/Annuity/IRA statement [1099-R]

-----Stock Dividends/Capital Gains [1099-DIV or Consolidated Brokerage Tax Statement]**

-----Stock or Bond sales [1099-B or Consolidated Brokerage Tax Statement]**

-----Real Estate Property Tax Bill for 2023 and record of property taxes actually paid in calendar year 2023, or rent paid for primary residence in 2023

-----Rent Certificate signed and dated by landlord (if applying for Homestead Credit)

-----Health Insurance Premiums paid in 2023, incl. Affordable Care Act [1095-A] or proof of exemption

-----Supplemental Health Insurance premiums paid

-----Information for Itemized Deductions (if applicable)

-----Miscellaneous Income [1099-MISC]

-----Federal and/or State SSI income received

-----All other forms or information as needed to complete your tax returns.

**These two forms may not be received until Feb.15 or later.  Schedule appointment accordingly.
